BlueOptions (PPO/EPO)

  • Available statewide
  • Largest physician and hospital network
  • Coverage for care outside of the network, except for Exclusive Provider Services
  • Specific providers required for prescription drugs and vision and/or dental services (if applicable)

BlueCare (HMO)

  • Available in select counties
  • Large physician and hospital network
  • No out-of-network coverage except for emergencies

BlueSelect (PPO/EPO)

  • Available in select counties
  • Limited hospital network
  • Coverage for care out-of-network, except for Exclusive Provider Services
  • Specific providers must be used for prescription drugs, durable medical equipment and supplies, home health, lab tests and vision and/or dental services (if applicable)


Not all plans are available in every county.

Florida Blue is a PPO, RPPO and Rx (PDP) Plan with a Medicare contract. Florida Blue HMO is an HMO plan with a Medicare contract. Enrollment in Florida Blue or Florida Blue HMO depends on contract renewal.
